    Foundations is a semi supported transitional accommodation for males and females aged 16-24 years old in the Blacktown LGA who are homeless or at risk of homelessness. The young people hold an initial 3 month lease with a community housing provider with the opportunity to stay for up to 18 months. We provide living skills programs and also offer client centered case management including but not limited to support with Centrelink payments, employment, family relationship breakdown, referrals to specialist services such as Drug & Alcohol counselling, psychology and grief and loss and the medical profession. Foundations also offers early intervention outreach support and case management to those at risk of homelessness by referral pathways and assisting to find crisis accommodation.
